Description

• Conduct thorough evaluations of patients in either inpatient or outpatient settings and create bespoke treatment plans
• Implement physical therapy interventions concentrating on mobility, strength, balance, and overall functionality
• Work together with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and their families on therapeutic objectives, discharge processes, and at-home exercises
• Maintain comprehensive documentation of evaluations, treatments, and progress in compliance with facility and regulatory requirements
• Engage in care team rounds and interdisciplinary meetings ensuring the coordination of holistic care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous, though not required

Skills

• Prior experience is highly desirable
