Forest Hills Queens Primary Care Center is looking for a dedicated Primary Care Physician to join our healthcare team in Forest Hills Queens. The successful candidate will play a vital role in delivering high-quality, patient-centered primary care services. This position involves diagnosing and managing a broad spectrum of health conditions, emphasizing preventive medicine and chronic disease management. You will collaborate with a skilled team of healthcare professionals to ensure comprehensive and coordinated care, promoting overall well-being for patients of all ages. Responsibilities
- Provide primary care services including diagnosis, treatment, and management of acute and chronic diseases.
- Conduct preventive health screenings and routine check-ups tailored to patients' needs.
- Establish and maintain ongoing relationships with patients to support health maintenance.
- Utilize electronic health records to document patient information accurately.
- Coordinate care with specialists and other healthcare providers to optimize patient outcomes.
- Educate patients on lifestyle modifications and disease prevention strategies.
- Participate in quality improvement initiatives and adhere to established clinical guidelines.
Requirements
- Medical degree (MD or DO) from an accredited institution.
- Board certification or eligibility in Family Medicine, Internal Medicine, or related specialty.
- Valid medical license to practice primary care medicine.
- Experience in outpatient primary care settings is preferred.
- Strong clinical decision-making and interpersonal skills.
- Ability to work effectively within a multidisciplinary team environment.
- Commitment to patient-centered care and ongoing professional learning.
