PRN Hospice Registered Nurse

Job Details Location: Marlton, NJ 08053 Position Type: Per Diem Salary Range: $1.00 - $1.00 Hourly Travel Percentage: Negligible Constellation Health Services is looking for a compassionate and skilled PER DIEM HOSPICE Registered Nurse to work with hospice patients and their families in their homes, assisted living facilities, and in skilled nursing facilities. Responsibilities Provides skilled nursing care including the administration of medications and treatments as prescribed in the physician's plan of care for patients in their home setting. Performs a comprehensive assessment and coordinates the plan of care with all disciplines associated with the patient's needs, including family and primary caregivers. Observes, evaluates and reports on the patient's physical and emotional status to the nursing supervisor and the attending physician whenever a change occurs. Participates in weekly Interdisciplinary Team Meetings, providing updates on patient's physical, emotional, psychosocial, and spiritual status. Maintains and processes all documentation in compliance with agency standards. Utilizes a computer-based documentation system and submits all required clinical documentation, time records, and other required information in the required time frame. Provides input to Home Health Aide Supervisor as needed for purpose of hospice aide evaluation. Weekend & holidays coverage (as needed). Registered Nurse licensed in the state of employment. At least 1-2 years of RN experience (must have). Previous hospice care strongly desired. Basic knowledge of iOS operating system. #J-18808-Ljbffr
