Skin Integrity Nurse $36.00 - $43.00 base pay Job Type: Full-time Qualifications Registered Nurse license in the state where the facility is located. Graduate of an accredited nursing program. Flexible with scheduling. Understanding of the integumentary system and willingness to continue learning. Responsibilities Complete weekly head-to-toe skin assessments and document in the EHR. Ensure appropriate treatments and/or interventions are in place for skin issues identified during skin rounds. Collaborate with the Clinician or Wound NP to obtain new orders or discontinue orders as needed. Update the ADON and DON on newly identified or worsened ulcers. Update the ADON and DON on non‑healing wounds. Verify and update resident care plans and documentation for identified skin impairments. Monitor skin‑integrity practices and procedures to ensure personnel implement the facility’s SIT program. Demonstrate proper infection‑control practices. Assist in evaluating and implementing recommendations from the Quarterly QA meeting. Ensure residents with known non‑compliance behaviors or high risk for pressure ulcers receive appropriate interventions. Attend and participate in weekly SIT meetings. Ensure residents’ responsible parties are updated on wound progress. Proactively develop positive employee relations and promote teamwork, mutual respect, and effective communication. Educate new and inexperienced nurse aides on proper methods of patient care. Perform other duties as requested by the DON or ADON. Possess and demonstrate a high degree of leadership, organizational ability, and communication skills. Utilize excellent communication skills to relate to patients, families, and staff members. Continuously update knowledge through ongoing education.
