Director of Social Work

Troy Victorian Rehabilitation & Nursing – Skilled Nursing Facility | 120 Beds

Hudson Wide Health Care is seeking an experienced and compassionate Director of Social Work to join our team at Troy Victorian Rehabilitation & Nursing.

This is an excellent opportunity for a motivated social work professional who is passionate about enhancing the quality of life for residents and supporting families throughout their care journey. The Director of Social Work will work closely with both Short-Term Rehabilitation and Long-Term Care residents , providing leadership, advocacy, and support while collaborating with our interdisciplinary team.

Key Responsibilities

• Conduct psychosocial assessments for new admissions

• Develop and implement individualized care plans

• Work closely with residents, families, and the interdisciplinary team to identify and address psychosocial needs

• Coordinate interdisciplinary care planning and discharge planning

• Assist residents and the care team with safe and effective discharge planning , including coordination of services and resources

• Complete timely and accurate documentation, including admissions, quarterly assessments, and MDS-related documentation

• Investigate, address, and document resident concerns and grievances

• Serve as a liaison between residents, families, and the care team

• Advocate for residents' needs, preferences, rights, and overall well-being

• Support residents and families through transitions of care and other significant changes in their care plan

Qualifications

• Bachelor's or Master's degree in Social Work or a related field

• Healthcare experience required; skilled nursing experience preferred

• Social Work licensure preferred

• Strong communication, organizational, and interpersonal skills

• Excellent problem-solving and documentation skills

• Ability to work collaboratively with residents, families, and an interdisciplinary team

• Compassionate, professional, and resident-focused approach
