Youth & Family Case Manager - Community-Based Care
Unison Health
Unison Health is seeking a dedicated Case Manager to coordinate services for youth and families living in low-income housing, with work conducted in client homes and in the community. You will provide Therapeutic Behavioral Services, document care, and collaborate with multiple partners to support clients in Toledo area settings.
