Job Summary:

Provide specialized physical therapy services to patients, supporting their recovery and improving functional mobility. Ensure treatment plans are effectively executed while maintaining compliance with licensing and facility standards.

Responsibilities:

• 
  Assess patients physical conditions and develop individualized treatment plans.

  Implement therapeutic interventions to promote rehabilitation and pain relief.
  
  
• 
  Monitor patient progress and adjust treatment approaches accordingly.
  
  
• 
  Document patient care activities accurately in accordance with regulatory requirements.

  Collaborate with healthcare team members to optimize patient outcomes.

  Educate patients and family members on therapy exercises and post-treatment care.
  
  

Qualifications:

• 
  Active state license as a Physical Therapist.
  
  
• 
  Proven experience in outpatient or clinical therapy settings.

  Strong knowledge of physical therapy techniques and modalities.

  Ability to interpret medical referrals and treatment plans.

  Excellent communication and patient education skills.
