RN Case Manager: Care Coordination & Outcomes

Santa Clara Family Health Plan

Santa Clara Family Health Plan is seeking an RN Case Manager responsible for delivering ongoing case management services to members. This role involves facilitating communication among care teams to promote effective outcomes. The ideal candidate will have an active California RN License, and at least three years of case management experience, with strong clinical knowledge and the ability to coordinate care within a team structure.
