Responsibilities
- Administer medications and treatments per physician orders and facility policy
- Monitor patient conditions, document vitals, and report changes to the supervising RN
- Assist with activities of daily living and support resident safety and comfort
- Perform basic nursing procedures and delegated tasks under supervision
- Maintain accurate medical records and charting in the designated system
- Collaborate with the interdisciplinary team to implement and update care plans

Requirements
- Active LPN license in Colorado
- CPR/BLS certification
- Prior experience in skilled nursing or long-term care preferred
- Ability to work night shifts and adapt to a fast-paced SNF environment
- Strong communication, documentation, and teamwork skills
- Proficiency with basic computer charting and electronic health records
