Registered Nurse-Home Care-FT (Ocean County)

Responsibilities Provide professional nursing care to patients in their home through assessment, development, implementation, and evaluation of home nursing care needs in accordance with organization policies and external regulations. Ensure compliance with physician orders and appropriate visit frequency; document progress toward discharge goals. Follow policy-driven assessment, planning, intervention, and evaluation of patient care; deliver holistic, safe care and perform treatments and procedures per standards; document all activities; utilize interdisciplinary team collaboration. Maintain professional standards of care and ethical practice; document clinical decisions appropriately within the system. Identify payors and maintain authorization timelines; work within authorization ranges and use secure messaging for authorizations. Document assessment, interventions, and planning; update care plans within two weeks of status changes; perform medication reconciliation and collaborate with physicians on the Plan of Care. Implement teaching strategies to meet care plan goals; document teaching activities in the medical chart consistent with goals. Report patient care problems promptly to management or physicians with possible solutions; utilize management as a resource. Act as liaison among physician, Manager of Clinical Practice, and other health care team members to ensure effective coordination and appropriate referrals. Communicate pertinent patient information and Plan of Care during transfers and on scheduled time off. Complete routine assignments accurately, independently, and in a timely manner; meet productivity requirements consistently. Supervise para-professional home health aide staff; perform supervisory visits within regulatory time frames and participate in staff evaluations. Stay current with updated standards and incorporate best practices into nursing care; document knowledge of current guidelines in medical records. Other duties and/or projects as assigned; demonstrate flexibility and adherence to organizational competencies and standards of behavior. Qualifications Education, Knowledge, Skills and Abilities Required Associate's Degree in Nursing or Diploma Certificate in Nursing At least six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice PC experience/skills Education, Knowledge, Skills And Abilities Preferred BSN Licenses And Certifications Required NJ State Professional Registered Nurse License AHA Basic Life Support (BLS) Healthcare Provider HMH is committed to pay equity and transparency for our team members.
