Roles & Responsibilities

• Conduct comprehensive initial evaluations of residents to identify physical limitations and develop individualized treatment plans.
• Implement evidence-based therapeutic exercises and interventions to improve mobility, strength, balance, and functional independence.
• Collaborate with nurses, physicians, and occupational therapists to ensure coordinated and holistic patient care.
• Educate patients and their families on safe mobility techniques, fall prevention, and home exercise programs.
• Accurately document evaluations, progress notes, and discharge summaries in compliance with facility and regulatory standards.
• Advocate for each resident's goals and celebrate their progress, fostering a positive and motivating rehabilitation environment.

Qualifications

• Active Physical Therapist license in the state of Pennsylvania (or willingness to obtain prior to start).
• Graduate of an accredited Physical Therapy program (new graduates are welcome to apply!).
• CPR/BLS certification required.
• Previous experience in skilled nursing, geriatric rehab, or outpatient orthopedics is a plus, but not required.
• Strong interpersonal skills and a compassionate, patient-centered approach.

• Patient assessment and functional mobility evaluation
• Therapeutic exercise prescription and implementation
• Manual therapy and balance/fall prevention techniques
• Clinical documentation and care planning
• Interdisciplinary collaboration and communication
• Ability to adapt treatment plans to diverse patient needs
