Hospice/Palliative Medicine Nurse Practitioner

Become part of a community hospice organization as a Nurse Practitioner, collaborating closely with the Medical Director, Hospice Physicians, and interdisciplinary team to deliver high quality, compassionate care to patients and families across diverse settings. The Position: Schedule is Monday Friday 8 am to 4:30 pm Provide direct clinical care including assessments, diagnosis, treatment planning, and follow-up care. Complete hospice face-to-face visits and eligibility evaluations. Provide care in the IPU and in patient homes. Conduct routine IPU rounding including symptom management and documentation. Prescribe medications and order treatments within scope of practice. Collaborate with physicians and interdisciplinary teams for coordinated care. Educate patients, families, and staff on hospice care. On-call coverage is required of at least one weekday per week from 4:30 PM to 7:00 AM and one weekend per month from 4:30 PM Friday to 7:00 AM Monday. Territory includes Manitowoc, Sheboygan, & Ozaukee County.
