Responsibilities
- Conduct initial and ongoing PT evaluations for residents in a skilled nursing care setting
- Develop, implement, and adjust individualized treatment plans to improve mobility, strength, balance, and functional performance
- Perform therapeutic interventions including gait training, gait analysis, ROM, strengthening, and balance activities; address ADLs
- Monitor progress, document outcomes, and maintain timely, accurate records in the charting system
- Communicate findings and collaborate with an interdisciplinary team (nursing, therapy colleagues, physicians) and provide caregiver and family education
- Reassess residents regularly, modify care plans as needed, and contribute to discharge planning and progress updates

Requirements
- Active Physical Therapist license in Massachusetts or eligible for licensure in MA
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
- Ability to work independently while collaborating with an interdisciplinary team
- Strong documentation and communication skills; proficient with standard electronic health records
