Title: RN Case Manager - Home Health

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 36

Requirements:
- Must be a licensed Registered Nurse in the state or hold an unencumbered compact license.
- Graduate of an accredited school of nursing.
- 2 years of experience in Home Health.

Roles & Responsibilities:
- Develop care plans in collaboration with patients, physicians, and caregivers.
- Periodically review and recertify patient care plans as appropriate.
- Coordinate patient care through interdisciplinary communication and case conferences.
- Provide ongoing assessments of patients' clinical, functional, and emotional status.
- Communicate significant changes in patient status to physicians and healthcare team members.
- Teach patients and caregivers self-care techniques and provide health counseling.
