Overview Position: Physician Assistant / PA-C Location: Golden Valley, MN (serving senior living communities in the Twin Cities Metro) Responsibilities Drive to Senior Living Communities to provide on-site primary care to residents Facilitate the patient's plan of care: chronic care management, advanced treatment, and end-of-life planning Coordinate with the interdisciplinary team, reconcile medications, and order labs Communicate clearly and regularly with Physician, office support staff, community staff, and patients' families Qualifications Current registration as a Physician Assistant / PA by the MN Board of Medical Practice Strong portfolio in Geriatrics, Primary Care, or Internal Medicine Passion for serving the Older Adult Population Strong interpersonal communication skills Valid driver’s license and current auto insurance Compensation & Benefits Compensation: $105,000 to $120,000 Benefits: Full Benefits Package About Twin Cities Physicians Established in 2016, Twin Cities Physicians serves over 4,500 residents within 200+ Senior Living Communities in Minnesota.
