Humana is seeking a Case Manager in Fort Atkinson to support members in navigating healthcare and community resources. In this role, you will assess medical, behavioral, and social needs; create person-centered care plans; and coordinate services across providers, payers, and community agencies. You will monitor progress, address barriers to care, and advocate for members to improve outcomes and reduce avoidable hospitalizations. Working in a collaborative, member-focused culture, you’ll leverage analytics, digital tools, and clinical guidance while engaging both in the community and via phone or virtual channels.
