PositionOverview: TheOncologyPharmacistwillensuresafe,optimalandefficaciousdrugtherapyforpatients,asyoureview,processandmonitorphysicianordersandpatientmedicationprofiles.Asanactivememberofthehealthcareteam,theOncologyPharmacistisresponsibleforcontactingotherhealthcareprofessionalsasappropriate,performingdutiesandresponsibilitiesinamannerconsistentwiththehospital’smissionandvalues. WhatYou'llDo: Dispenseandpreparespecializedmedicationsforpediatriconcologyandhematologypatientsandcollaborateacrossdisciplinestoshapeandexecutepatienttreatmentplans. Verifymedicationordersforaccuracy,appropriateness,andpotentialinteractionsandeducateandcounselpatients,families,andcaregiversonmedicationuse Monitordrugtherapyforeffectivenessandadverseeffects. Contributetopatientcarethroughactiveparticipationinclinicalrounds. Managechemotherapyprotocolsandsafehandlingofhazardousdrugs. Performmedicationreconciliationduringcaretransitionsandprovideexpertdruginformationandconsultations. Supportthedevelopmentofpharmacypoliciesandproceduresandensureadherencetoallregulationsandsafetystandards. Engageinqualityimprovementandresearchinitiatives. Maintainprecisepatientandmedicationrecords. Staycurrentwithadvancementsinpediatrichematology/oncologypharmacotherapy.
