Physical Therapist

Schedule: Tuesday - Saturday, 8 AM - 5 PM

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 36

Requirements:

• Vermont PT licensure
• Home Health experience
• OASIS experience
• McKesson/Netsmart EMR experience preferred

Roles & Responsibilities:

• Assess patient conditions and develop individualized plans of care to maximize mobility and independence.
• Collaborate with an interdisciplinary team to ensure patient needs are met.
• Perform standard therapy measurement techniques, including ROM, strength, and balance tests.
• Utilize various therapeutic modalities to provide optimal care to clients.
