Physical Therapist (PT) – Full-Time
Agawam Campus
Agawam, MA
$45–$50/hour

Post Rehab is seeking a Physical Therapist (PT) to join our rehabilitation team within a skilled nursing facility. This role offers the opportunity to work closely with residents in both short-term rehab and long-term care while helping improve mobility, strength, balance, and functional independence.

Position Responsibilities:

• Complete physical therapy evaluations and establish individualized treatment plans
• Provide therapy focused on gait training, transfers, balance, strengthening, endurance, and pain management
• Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support resident goals
• Monitor resident progress and adjust treatment plans as clinically appropriate
• Maintain timely and accurate documentation in accordance with facility standards

Qualifications:

• Active Physical Therapist license required
• Strong communication and clinical reasoning skills
• Skilled nursing experience preferred but not required
