Physical Therapist – Skilled Nursing Facility

Schedule: 08:00:00 - 16:30:00, M-F, Weekend rotation

Assignment Details:

• Contract Length: 52 weeks
• Guaranteed Hours: 0

Requirements:

• License: Physical Therapist (PT)
• Certifications: Heart Failure Certification (AHA) is preferred

Roles & Responsibilities:

• Conduct assessments and develop individualized treatment plans for patients.
• Implement therapeutic techniques and procedures to help patients improve mobility and function.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Educate patients and their families on treatment plans and home exercises.
• Maintain accurate and up-to-date documentation of patient progress and outcomes.

Compliance / Notes:

• Orientation and onboarding processes will be provided.
• Background checks and drug screening are required.
• RTO policies as per facility guidelines.
