Health Services Coordinator (LPN) Full‑time, 40 hours/week. Sunday – Thursday. Pay DOE. Job Summary The Health Services Coordinator supports the health services team, preserving dignity and promoting independence for each resident while delivering care and services according to individual service plans and MBK policies. Duties & Responsibilities Conduct and coordinate assessments/evaluations of potential residents and recommend admissions based on current regulations and policies. Draft and update initial individualized Service Plans as needed. Review service plans with responsible parties and the Executive Director as requested by the Director of Health Services. Ensure continuity of assisted living residents’ total care regimen. Provide training and education on medication delivery, safe practices, emergency procedures, record keeping, and state requirements. Perform all assigned duties accurately and timely, including required documentation. Serve as a medication technician when required. Provide coverage during absences, either by arranging backup or completing duties personally. Check vital signs and monitor health status changes. Perform basic nursing functions such as treatments, medication delivery, and managing resident emergencies. Maintain adequate inventory of resident care supplies, minimizing waste. Coordinate prescription orders with doctors’ offices and manage pharmacy delivery, including central storage oversight and medication administration record completion. Aid in community marketing through positive interactions, acting as a liaison between the community, families, and outside providers. Maintain a safe and secure environment for staff, residents, and guests following safety standards and policies. Understand and comply with all federal, state, and local regulations and company policies. Promote teamwork and open communication in line with MBK principles and core values. Perform other job duties or special projects as assigned by the Supervisor or Executive Director. Education Requirements Active RN, LVN, or LPN license in good standing, including completion of required CEUs and timely renewal. Experience Requirements 2+ years of prior related work experience in a similar healthcare environment. Required Competencies, Licenses & Certifications Complete background clearance, health screening, and provide a negative TB test within 7 days of employment (valid within the last 6 months). Valid state driver’s license and reliable transportation. Competent technical and computer skills; familiarity with Microsoft Office Suite (Word, Excel, Outlook) and office equipment. Excellent communication skills in speaking, writing, and reading English. Sound independent decision‑making ability and calmness under stress or emergencies. Tactful and professional interaction with personnel, residents, families, and guests. Strong organizational, problem‑solving, and time‑management skills. Maintain a neat and organized work environment to promote safe resident care coordination. Physical Demands & Work Environment Flexible schedule, including weekends and holidays. Mobile and able to perform walking, bending, kneeling, squatting, pulling, reaching overhead, and repetitive motions. Intermittent movement throughout the community and between neighborhoods. Lift and carry up to 50 pounds; push up to 250 pounds. Assist in transferring residents during routine or emergency situations. Benefits Impacting lives and building lasting relationships.
