Developmental Disability (DD) Case Manager (ID294) The Intellectual Disability (ID) Case Manager serves to assist in the provision of services to intellectually disabled individuals of the jurisdiction(s) served by his/her clinic site position location. Primary duties include individual assessment, service plan development, residential placement and follow-up, information and referral, case recording, and liaison with other agencies and facilities which serve intellectually disabled individuals. This job description is based upon an evaluation of agency requirements at the time this description was written. MAJOR DUTIES AND RESPONSIBILITIES Assesses individual’s comprehensive needs for services and supports through interviewing individual and/or family, and utilizes appropriate assessment tools. Develops individualized written case management services plans which outline service needs. Links individuals to services and supports specified by the service plans. Maintains complete and accurate individual records, in accordance with all applicable regulations. Coordinates, monitors, and advocates for individuals’ receipt of required services from various service providers. Provides supportive intervention and follow-up with individuals and families. Administers the Universal Assessment Instrument at assisted living facilities or specially funded locations for those individuals that targeted case management is provided. Serves as resource on Developmental Disabilities Services to other Community Services Board personnel and community agencies. Develops and submits required statistical data for reporting purposes. Provide DD services in compliance with Medicaid and licensure guidelines. Assess individuals for inclusion on the statewide DD Waiting List. Participate in clinic DD staffing and other clinic staffing as needed. Provide DD consultation for Emergency Services staff and DD providers when needed. Attend Department of Behavioral Health (DBHDS)-ODS DD Medicaid Waiver training updates, ID Services Quality Service Team (QST) meetings, and Family Assessment and Planning Team (FAPT) meetings for DD cases when needed. Process DD Non-Waiver Program participants. Timely completion of accurate time sheets, credible services, and other assigned paperwork. Performs other duties as assigned by the supervisor QUALIFICATIONS A combination of mental health work experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required. Bachelor’s degree earned from an accredited college or university in a relevant human services field required. At least one year of experience working in DD services to meet Department of Medical Assistance (DMAS) requirements required. Must possess a valid driver’s license, a good driving record, and the ability to provide your own transportation. ADDITIONAL REQUIREMENTS May be exposed to allergenic environments in client’s homes, including extreme heat, humidity, dust, smoke, pet contamination, cleaning supplies, etc. May be exposed to clients with infectious diseases. Must be able to drive for long periods of time to visit clients in and out of the catchment area as needed. Must be willing and able to assist clients with basic duties such as shopping, carrying personal items and putting household items away. Must be able to walk distances, climb and descend stairs, maneuver in and out of Agency vehicles. Prolonged periods sitting at a desk and working on a computer. Must be able to lift/push/pull up to 25 pounds at times.

SALARY: $52,788 – $54,637
