Care Manager

The Care Coordinator has the responsibility for coordinating the care and services for assigned patients for an episode of illness or treatment. This coordination is done in collaboration with the patient, family, physician, patient care team, and payors. The Care Coordinator utilizes advanced clinical skills to facilitate the provision of care. This includes the assessment, planning, intervention, and evaluation principles of the nursing process. The Care Coordinator promotes and evaluates the effective utilization of resources by using current clinical knowledge and awareness of community services. They assume a leadership role with the patient care team to achieve optimal clinical and resource outcomes. The Care Coordinator evaluates and analyzes quality and utilization cases, admission and discharge criteria, and readmits within 30 days. They participate in outlier review on an ongoing basis. In addition, the Care Coordinator may be assigned to the Emergency Department. In collaboration with the patient and family, and the multidisciplinary team including physicians and payors, they ensure the patient's progress along the emergency episode of care. This is done in an efficient and cost-effective manner for the assigned case load. The Care Coordinator advocates for the patient and family throughout the entire episode of care. They maintain availability to the patient and family as a resource. This facilitates communication among providers and monitoring of services rendered. The Care Coordinator remains involved until the patient is discharged or appropriately transferred to an acute care setting or an alternate level of care. They initiate appropriate clinical pathways based on admission diagnosis and serve as a resource for utilization management purposes.
