Pediatric Inpatient RN Case Manager - Discharge & Care

AMN Healthcare is seeking an experienced RN Case Manager to coordinate patient care across the continuum in Syracuse, NY. You will manage, coordinate, facilitate, and monitor resources to achieve optimal clinical outcomes, utilization of services, and financial goals. You will oversee the discharge process for all patients within the unit/core service area and support a safe, appropriate discharge plan.
