Social Worker – Boston Children’s Health Physicians – Valhalla, NY The Pediatric Nephrology and Rheumatology Divisions of Boston Children’s Health Physicians are seeking a full‑time social worker to support our growing team of physicians. The role involves caring for children with chronic kidney disease who require dialysis and kidney transplantation. Duties include mental health screening, service referrals, case management, building rapport with families, and addressing social factors such as transportation, housing, insurance, and schooling. The candidate will collaborate with our team, families, community agencies, and coordinate clinical services. Advocacy for families within various resource systems and navigation of service organizations to complete referrals is essential. Individual and/or family counseling for children, adolescents, and adults is encouraged, depending on the credentials of the successful candidate. Conversational Spanish is preferred but not required. Supervision for clinical licensure is available. LMSW or LMHC required. Pay rate will be $25/hr – $39/hr, negotiable based on experience.
