Description: Admit-to-Discharge Therapist – PRN $55/Hour | Outpatient + Home Care | Flexible Weekday Schedule

Community Physical Therapy (CPT) is looking for a PRN therapist to join our Admit-to-Discharge team! If you're looking for flexibility, a strong team environment, and the opportunity to work with patients from evaluation through discharge, this could be a great fit.

• Monday–Friday flexibility, with scheduling between 7am–6pm

What we're looking for:

• Active Illinois therapy license
• Outpatient or home care experience is a plus
• Someone dependable, flexible, and comfortable working independently
• Willingness to learn TheraOffice and Homecare Homebase
