Practice Setting and Clinical Scope

In this clinic-based role, you will deliver comprehensive internal medicine care while utilizing evidence-based guidelines and value-based principles. Engage in efforts to improve patient outcomes and reduce healthcare disparities through the use of population health tools, risk stratification, and care coordination strategies. Collaborate closely with care managers, pharmacists, social workers, behavioral health specialists, and community partners to provide coordinated, team-based care.

This is a full-time, direct hire position offering a daytime clinic-based schedule. Qualifications

• Experience in medical education, health equity, and population health is highly preferred.
• Ability to work collaboratively in a multidisciplinary care environment is essential.
