Position Overview:

As an Advanced Practitioner with CCP, you will provide direct patient care in a collaborative environment that integrates primary and specialty services. You will practice to the full extent of your license, supported by a multidisciplinary team dedicated to delivering comprehensive, high-quality healthcare.

Responsibilities:

• Perform patient assessments, establish diagnoses, and develop individualized treatment plans.
• Order, interpret, and follow up on diagnostic studies.
• Prescribe medications and manage acute and chronic conditions.
• Provide counseling and education to patients and families on preventive care and wellness.
• Collaborate with physicians, nurses, and other members of the care team to ensure continuity of care.

Qualifications:

• Current New York State NP or PA license.
• National certification (or eligibility).
• Strong clinical, communication, and teamwork skills.
