Job Summary:

Responsibilities:

• 
  Administer medications and treatments in accordance with care plans.
  
  
• 
  Monitor and document patient vital signs and condition changes.
  
  
• 
  Collaborate with interdisciplinary team to develop and update care plans.
  
  
• 
  Educate residents and families on health management and treatment options.
  
  
• 
  Maintain accurate and timely documentation of patient care activities.
  
  
• 
  Adhere to facility and regulatory policies, including proper identification and safety protocols.
  
  

Qualifications:

• 
  Valid state nursing license (RN).
  
  
• 
  Proven experience in LTC/SNF or similar long-term care settings.
  
  
• 
  Ability to maintain professionalism and reliability under pressure.
  
  
• 
  Knowledge of state and federal healthcare regulations.
  
  
• 
  Demonstrated ability to work independently and as part of a team.
  
  
• 
  Must be able to bring a valid ID and comply with vaccination requirements or hold applicable exemptions.
