RN- Case Manager - (Candidate in TBC)

WCS Healthcare is seeking a compassionate and experienced Registered Nurse (RN) Case Manager to support adults with intellectual and developmental disabilities (I/DD) in residential and day program settings throughout Carmel, Patterson, Brewster, and Pawling, NY .

This is a primarily case management and oversight role , ideal for an RN who enjoys coordinating care, supporting staff, and ensuring high-quality services. This position does not involve routine hands-on bedside care.

 

Schedule

• Full-Time
• Monday-Friday
• 9:00 AM - 5:00 PM
• Flexible scheduling available within daytime hours
• Nurses receive schedules in advance and will report to one location per day
• No travel between multiple sites during the same workday

 

Responsibilities

• Provide nursing oversight for residential and day program services serving adults with developmental disabilities
• Supervise and support Direct Support Professionals (DSPs)
• Develop, coordinate, and monitor individualized care plans
• Ensure compliance with OPWDD regulations and agency standards
• Review and maintain accurate clinical documentation
• Assess healthcare needs and collaborate with interdisciplinary teams
• Serve as a clinical resource for staff, individuals served, and families
• Monitor health status and coordinate follow-up care as needed

 

Work Environment

• Support approximately two residential homes and one day program as assigned
• Each residence typically serves 5-10 individuals
• Travel between assigned locations throughout the week based on program needs
• Collaborative and mission-driven environment focused on enhancing quality of life

 

Qualifications

• Active New York State RN License required
• Experience working with OPWDD, I/DD, developmental disabilities, residential programs, day habilitation, or case management required
• Strong documentation and organizational skills
• Ability to work independently and manage multiple priorities
• Valid driver's license and reliable transportation required
• Available to work full-time Monday through Friday

 

Training

Comprehensive onboarding includes:

• Medication administration training
• PONS reporting system training
• Litmos/Relias online training
• New York State OPWDD training
• Approximately 2 weeks of training (may be completed sooner based on experience and progress)
