SUMMARY/OBJECTIVES
The Nurse Practitioner (NP) works collaboratively with the Attending Physicians and the Chief Medical Officer to provide comprehensive, evidence-based healthcare services to patients in accordance with applicable federal, state, and local laws, regulations, accreditation standards, and organizational policies.
The Nurse Practitioner is responsible for the assessment, diagnosis, treatment, and ongoing management of patients across the lifespan while promoting patient-centered care, disease prevention, health promotion, and chronic disease management. The NP practices independently within the scope of licensure and collaboratively with physicians and interdisciplinary healthcare professionals to ensure high-quality clinical outcomes.
This position may provide services at multiple Edgewater Health locations, including Primary Care Clinics, Rapid Access Center (RAC), Turning Point, School-Based Health Centers, and other assigned service locations. Participation in telehealth services, chronic care management, and on-call coverage is required as assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Patient Assessment and Evaluation
Conduct comprehensive physical examinations and obtain complete medical, social, family, and medication histories.
Assess patient symptoms and determine appropriate diagnostic and treatment interventions.
Order, perform, and interpret laboratory tests, diagnostic imaging, and other diagnostic procedures.
Evaluate patient health status and formulate appropriate differential diagnoses.
Perform comprehensive health assessments for preventive and chronic disease management.
Provide on-call coverage as assigned, including coverage for multiple Edgewater Health service locations.
Diagnosis and Treatment
Diagnose and treat acute illnesses, chronic diseases, and injuries.
Develop individualized treatment plans based on evidence-based clinical guidelines.
Prescribe medications and therapeutic interventions consistent with Indiana Nurse Practitioner prescribing authority.
Perform minor office procedures within the scope of practice.
Monitor treatment effectiveness and adjust care plans as clinically indicated.
Coordinate continuity of care through appropriate follow-up appointments.
Patient Education and Counseling
Educate patients and families regarding diagnoses, treatment options, medications, and preventive care.
Counsel patients regarding medication compliance, lifestyle modifications, nutrition, exercise, smoking cessation, and disease prevention.
Provide education regarding chronic disease self-management.
Encourage patient participation in treatment planning and healthcare decision-making.
Collaboration and Care Coordination
Collaborate with physicians, behavioral health providers, specialists, nursing staff, case managers, pharmacists, and other healthcare professionals.
Coordinate referrals to specialty care, diagnostic services, and community resources.
Participate in multidisciplinary treatment planning and case conferences.
Ensure seamless transitions of care through effective communication among providers.
Health Promotion and Disease Prevention
Perform preventive screenings and wellness examinations.
Administer immunizations, injections, and preventive treatments.
Provide chronic care management services, including telehealth appointments.
Promote healthy lifestyles through patient education and risk reduction counseling.
Identify patients requiring preventive interventions and coordinate appropriate care.
Documentation and Compliance
Maintain complete, accurate, and timely documentation within the Electronic Medical Record (EMR).
Ensure all documentation meets legal, regulatory, payer, and accreditation requirements.
Maintain patient confidentiality in accordance with HIPAA regulations.
Comply with all federal, state, local, and organizational regulations governing clinical practice.
Participate in quality improvement initiatives, peer review activities, and performance improvement projects.
Maintain required professional licensure, certifications, continuing medical education (CME), and credentialing requirements.
Follow all organizational clinical protocols, infection prevention standards, and patient safety initiatives.
Organizational Responsibilities
Participate in departmental meetings, staff meetings, committees, and organizational initiatives.
Maintain professional competency through continuing education and professional development.

Travel between clinic locations as assigned.

QualificationsMINIMUM QUALIFICATIONS
Education: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution.
Licensure Requirements: Current unrestricted Indiana Registered Nurse license.
Current Indiana Nurse Practitioner license/certification issued by the Indiana State Board of Nursing.
National Board Certification as a Nurse Practitioner appropriate to specialty practice.
Current DEA registration and controlled substance prescribing authority (or ability to obtain).
Current Basic Life Support (BLS/CPR) certification.
CPI Certification or ability to obtain within required timeframe.
Valid Indiana driver's license meeting Edgewater Health motor vehicle requirements.
Ability to travel between assigned clinical locations.
Ability to successfully complete credentialing and privileging requirements.
PREFERRED QUALIFICATIONS
Minimum of two (2) years of Nurse Practitioner experience in primary care, family medicine, community health, behavioral health integration, emergency medicine, or related specialty.
Experience providing care within a Federally Qualified Health Center (FQHC), Community Mental Health Center (CMHC), or integrated healthcare setting.
Experience utilizing Electronic Medical Record (EMR) systems.
Experience providing telehealth services.
Bilingual language proficiency.
Experience with value-based care and chronic care management programs.
SUPERVISORY
Reports To: The attending Physician and Medical Director
Supervise: This position does not have direct supervisory responsibilities. The Nurse Practitioner is expected to provide clinical leadership, mentorship, and professional guidance to nursing staff, medical assistants, students, and other clinical personnel as appropriate.

WORKING CONDITIONS:
Employment is contingent upon successful completion of all pre-employment requirements, including:
Tuberculosis (TB) screening prior to employment and annually thereafter.
Documentation of required immunizations, including MMR, Varicella, Influenza, COVID-19, and other vaccinations as required by organizational policy.
Successful completion of pre-employment drug screening and any reasonable suspicion drug or alcohol testing thereafter.
Successful completion of criminal background investigation and reference verification.
Completion of all required employment documentation and organizational orientation prior to beginning work.
Successful completion of department orientation within six (6) months of employment.

Completion of HCI Quality Training, Trauma-Informed Care, Compliance Training, HIPAA, Infection Prevention, and all required e-learning modules.
Attendance at all mandatory staff meetings and required continuing education programs.

Flexibility to work varying schedules, including evenings, weekends, holidays, rotating shifts, and on-call assignments as operational needs require.
