Physical Therapist

Provides medically prescribed clinical physical therapy in patients' homes. Primary position responsibilities include facilitating and coordinating physical therapy care, evaluating progress toward attainment of patient outcomes, documentation of care, employing strategies to promote a safe environment and patient health, and education of the patient and caregiver. Other duties as assigned.

Qualifications:

Minimum education: Required: Bachelor's Degree in Physical Therapy. Preferred: Master's or Doctoral Degree in Physical Therapy.

Minimum years of experience (amount, type and variation): Required: Two years of clinical experience in a hospital, out-patient, or rehab setting. Preferred: One year of home health experience.

License, registry or certification: Required: Current license to practice Physical Therapy in the State of New Jersey. Valid driver's license. Current CPR certification. If not certified, must obtain certification within six (6) months of hire. Preferred: None.

Knowledge, skills and/or abilities: Required: Excellent communication, time management, judgment and decision making skills. Computer functionality knowledge. Knowledge and skill in current PT practice. Preferred: Knowledge of home health care provision and principles.
