Social Worker II (Intermediate) — On-Site | Erie, PA (16507) | 40 hrs/week (Contract) Pay rate: $22-$25 an hr Schedule Options Tue–Sat or Mon–Fri Shifts may include:7:30a–3:30p, 8:00a–4:00p, 9:00a–5:00p, or 10:00a–6:00p Location:Erie, PA 16507 (On-site) Schedule:Flexible —Tue–Sat or Mon–Fri(multiple 8-hr shift options) Hours:40 hours/week Start:ASAP Employment Type:Contract to hire Position Summary We are seeking aSocial Worker II (Intermediate)to provide psychosocial assessment, care planning support, and ongoing social services for residents/clients in an on-site healthcare setting. This role collaborates closely with an interdisciplinary team to support safe care transitions, connect clients to resources, and address barriers impacting health and wellbeing. Key Responsibilities Completepsychosocial assessmentsand gather relevant social/behavioral history. Provideshort-term counseling, emotional support, and crisis interventionas needed. Participate ininterdisciplinary care planning, ensuring psychosocial needs are reflected in the plan of care. Coordinatedischarge planning / transitions of care, including referrals to community resources and follow-up services. Assist withresource connection(benefits, housing/community programs, transportation, caregiver support, etc.). Supportfamily communicationand facilitate meetings when appropriate. Maintain timely, accuratedocumentationin accordance with facility policy and applicable regulations. Advocate for clients/residents and help addresssocial determinants of healthimpacting outcomes. Work collaboratively with nursing, providers, case management, and other partners to reduce barriers to care.
