Torrington Community Hospital is a 25-bed critical access hospital serving residents in southeast Wyoming and bordering Nebraska communities. The hospital is located about 1 hour from Cheyenne and near the Black Hills, Rocky Mountains, and Denver, offering rural living with outdoor and cultural opportunities. Torrington Community Hospital is looking for a full-time Registered Nurse to work on the Medical Surgical unit in Torrington, Wyoming. This position is 36 hours per week, nights from 6:00pm – 6:30am. Position Summary This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice. The nurse is accountable for the quality of nursing services delivered by self or others under their direction and uses specialized knowledge, judgment, and nursing skills to care for patients across developmental ages. Core Responsibilities Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews history/physical with patient and family and coordinates with the health care team. Reviews diagnostics and reports abnormal results to the physician and care team. Completes assessment/reassessments as needed and per policy. Formulates a plan of care, including discharge planning, utilizing assessment data and input from patient, family and health team. Sets realistic, measurable goals in collaboration with patient/family. Plans care with the multidisciplinary team and updates as patient needs change. Implements the plan of care through direct patient care, coordination, delegation and supervision of the health care team. Follows physician orders and policies, delegates appropriately, and communicates patient status to caregivers to ensure continuity of care. Evaluates patient and family response to care and teaching; revises the plan of care as needed. Monitors progress toward goals in collaboration with the health care team. Documents assessment, planning, implementation and evaluation in the patient record. Ensures documentation is legible, timely and reflects data, interventions and patient response. Notes physician orders accurately and timely. Provides care based on the best available evidence and may participate in research. Participates in unit or facility leadership, education, role development and orientation of staff, students, families and visitors. Supports growth through precepting and mentoring when appropriate. Contributes to patient outcomes through timely, effective, efficient, equitable and safe care. Participates in improvement activities and may engage in professional organizations to advance nursing quality indicators. In some roles, may supervise staff and department workflow. Interacts with staff, physicians, patients, families and external contacts to coordinate care and report/clarify information. Collaborates with other health care providers and, when appropriate, with clergy for spiritual support. Synthesizes and prioritizes data from multiple sources to support the human response of patient and family to changes in health status. Minimum Qualifications Must possess a current, valid RN license in the state of practice, a temporary/interim RN license, or compact licensure for the current state of practice. BLS certification required. Additional certification or continuing education may be required based on area of practice. Experience in the clinical area is desired. Must maintain clinical performance competencies demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year of recent acute care hospital RN experience; experience in the past 12 months in an acute care setting is preferred. In Home Health, ability to operate a personal vehicle and maintain a valid driver’s license; auto insurance must be active.
