Position Overview:
We are seeking an experienced PRN Physical Therapist to join our growing team. This role will primarily involve providing physical therapy services within assisted living and residential care settings. The ideal candidate is confident, independent, and experienced in the home health environment, including documentation and compliance standards.

This is an excellent opportunity to get in on the ground floor of a startup agency, with strong potential to transition into a full-time role as the company becomes fully established.

Key Responsibilities:

• Perform physical therapy evaluations and develop individualized plans of care
• Provide skilled therapy interventions to improve mobility, strength, balance, and overall function
• Collaborate with nursing staff, caregivers, and interdisciplinary team members
• Complete accurate and timely documentation in compliance with home health regulations
• Educate patients and staff on safety, mobility techniques, and fall prevention
• Participate in care coordination and communication with physicians and other providers

Qualifications:

• Licensed Physical Therapist in the state of Kansas
• Required: Prior experience in home health physical therapy
• Strong knowledge of home health documentation standards and compliance requirements
• Experience with WellSky preferred but will train
• Ability to work independently and manage time effectively
• Excellent communication and clinical assessment skills

Pay = $65/routine visit and $90/evaluations/SOC/Recerts/Discharge. $67/hr for required training, QAPI meetings, etc.
