Most importantly, our colleagues are appreciated for the real differences they make in both the lives of their clients and their clients' families.CNA Job Responsibilities:Provides personal care and housekeeping assistance to our clients in their homes; follows the established plan of care under the direction of the client care coordinator.Provides personal hygiene and grooming care to our clientsProvides meals for clients' nutritional needsReports changes in clients' condition to client care coordinator or supervisorPerforms routine housekeeping and home management dutiesRecords each client's care on care plan and submits care plan in a timely mannerResponds sensitively to the multiple, complex needs of a terminal client which includes providing end of life care.Assists with routine therapy exercisesParticipates in case conferences as requestedQualifications:Education: High School diploma or equivalent, preferredLicensure/Certification: CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services requiredExperience: Six months experience as Home Care Aide/CNA in a home care agency, preferredWith locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
