Care Transition Coordinator

VitalCaring is seeking a Care Transition Coordinator to build and manage referral relationships that connect patients to our home health and hospice services. In this sales-focused role, you will partner with hospitals, physicians, and discharge planners to educate on VitalCaring’s programs, coordinate timely transitions to home, and grow referrals in an assigned territory. You’ll blend relationship-building, clinical understanding, and strong communication to support patients and families, while working with a collaborative, mission-driven team that values compassion, excellence, and work–life balance.
