Registered Nurse (RN) - Case Manager

RN – Case Management

Schedule: 4 x 8-hour day shifts Every other weekend

Assignment Details:

• Contract length: 13 weeks
• Guaranteed hours: 72 hours per 2-week pay period

Requirements:

• RN licensure (pending license accepted)
• Minimum of 1 year acute care experience
• Experience in discharge planning / care coordination
• Strong preference for case management experience across Med-Surg, LLOS, and ED
• Experience with patient ratios of approximately 25–30 patients
• Proficiency in EPIC
• Basic Life Support (BLS) certification required

Roles & Responsibilities:

• Coordinate patient care and facilitate discharge planning to the next level of care.
• Conduct needs assessments and develop individualized plans of care for patients.
• Collaborate with interdisciplinary team members, including social services, to ensure comprehensive patient care.
• Manage care transitions and provide support for patients across all age groups, from neonates to geriatrics.

Compliance / Notes:

• Orientation: Approximately 30 hours
• Candidates will float across similar units and between campuses
• Must commit to working 3 of the following holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day
• Facility paid parking available
• Scrub color: Any allowed except reddish orange
