About This Role The Home Care Registered Nurse provides comprehensive nursing care to a designated group of patients, utilizing the nursing process to assess, plan, implement, and evaluate patient care. This role is ideal for nurses who excel in collaboration and leadership within a home care setting. Highlights Location: Manhasset, NY Schedule: Full-Time, Day shift Sign-On Bonus: $10,000 Student Loan Forgiveness Plan or $10,000 Retention Bonus Program What You’ll Do Conduct patient interviews and explain policies and procedures. Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status. Communicate and document patient’s physiologic health status and plan of care. Assess patient’s psychosocial health status and identify teaching needs. Develop criteria for measurement of patient outcomes and establish individualized patient goals. Coordinate the cost‑effective use of supplies, equipment, and medication. Implement the plan of care and maintain patient safety precautions. Administer prescribed treatments and provide emotional support to patients. Keep accurate documentation of patient’s treatment and responses. Participate in planning for discharge and coordinate referrals. Requirements Education: Graduate from an accredited School of Nursing required Bachelor’s Degree in Nursing preferred Experience: Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date Licenses & Certifications: Current License to practice as a Registered Professional Nurse in New York State required Valid Driver's License in New York State may be required depending on area of assignment #J-18808-Ljbffr IntelliResume
