This includes assessing patient health needs, developing nursing care plans, and educating patients on relevant health management information. This position also oversees the operational running of the medical clinical area and patient communication within the electronic health record system.Main Responsibilities:Clinical Floor OversightPrepare clinical area for building opening or closing in accordance with Finger Lakes Community Health hours of operationRedirect nursing team tasks to ensure an efficient patient flowComplete medication reconciliationEnsure the following duties are completed by nursing staff: Pre-visit planningInventory restock and orderingQuality Control measuresEquipment inspection date checks Sterilization of instruments, equipment, and clinical areasTeach nursing staff on use of equipment, techniques, and clinical servicesPerform preventative maintenance on medical equipmentClinical ServicesAssist with minor procedures and exams as outlined in NYS clinical scope of practicePerform patient screenings and assessments as outlined in NYS clinical scope of practiceComplete phlebotomy and lab collection and handling as outlined in NYS clinical scope of practiceConfirm patient charts have written orders for medications and/or injections from the providerAdminister injections and immunizations as directed by ordering providerReview and send medication refill requests to patient's providerInform patients or designated persons of physician orders, changes in care plan, test results, etc. as outlined in NYS clinical scope of practiceCare ManagementAttend morning huddle meeting with patient care team to address patient concernsReview specific populations health care needs and follow-up appropriatelyOversee referral process for patients as directed by provider including initiation, follow-up, and documentationEducate patients and family members on health information and medication administrationPromote patient's independence by establishing patient care goalsServe as liaison between patient and provider when neededMedical Record MaintenanceChart services in patient record including but not limited to: Referrals, medications, vaccinations, injections, screenings, lab orders/results, patient care goals, patient history, vitalsAudit progress notes to ensure all patient history and vital signs have been taken and noted by nursing staffChart and lock notes within timely manner in accordance with Finger Lakes Community Health policies and proceduresAddress telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and proceduresEducation and Qualifications :A current, unrestricted NYS Registered Nurse License requiredCPR certification required (can be provided)1-3 Year of experience working in licenseThe ability to work flexible hours -- some evenings are requiredA valid government-issued photo identification document is requiredPhysical Requirements:Must be able to hear and communicate with clients and staff via phone, web media, and in person.Must be able to lift up to 40 lbsMust be able to sit for extended periods of timeMust be able to kneel, crawl, or bend into awkward spacesOSHA Level 1Finger Lakes Community Health uses BambooHR for the hiring and onboarding of our staff.
