Key Responsibilities Conduct detailed assessments to understand the specific needs of clients and develop personalized treatment plans that foster their emotional and psychological well-being. Implement evidence-based therapeutic interventions tailored to hospice patients, focusing on alleviating emotional distress and promoting family support. Maintain rigorous clinical documentation, ensuring that all records are accurate, confidential, and compliant with industry regulations. Collaborate with our multidisciplinary treatment team, including medical professionals and other therapists, to provide holistic care for clients. Educate and offer resources to patients and their families, equipping them with coping strategies during the hospice experience. Commit to ongoing professional development by staying informed about the latest mental health research and therapeutic modalities. Patient Population / Client Focus Your primary focus will be individuals receiving hospice care, along with their families. This unique population requires a sensitive approach to therapy as they face significant emotional and psychological challenges associated with terminal illnesses. You will provide support and interventions to help clients process grief, anxiety, and fear, fostering a supportive environment where they feel valued and understood. Qualifications A Master's degree in Social Work from an accredited institution. A valid LCSW license in the state of Oregon. Minimum of two years of clinical experience in a mental health setting, preferably with end-of-life care. Excellent proficiency in clinically relevant techniques, including solution-focused and trauma-informed therapy. Strong interpersonal and communication skills, with a natural ability to build rapport with patients and families. Exceptional problem-solving skills and critical thinking abilities, enabling you to navigate complex emotional dynamics. A collaborative spirit, thriving in a team-oriented environment. Preferred Experience Experience working in hospice or palliative care is highly preferred, along with familiarity with chronic illness and the unique challenges faced by patients and their families in these circumstances. Proficiency in using digital health tools for teletherapy and patient management is advantageous.
