Assures safe and effective use of medications through clinical involvement. Participates in education activities related to drug therapy to enhance personal knowledge as well as that of other health professionals. Participates in administrative, medication use evaluations, and other performance improvement activities. Involved in direct patient education and interviews to obtain an accurate medication list. Child (1 year through 12 years); Adolescent (13 years through 17 years); Adult (18 years through 65 years); Geriatric (over 65 years) Pharmacy Specific Duties and Responsibilities Responsible for medication use control Assures safe and effective use of medications through clinical involvement Participates in educational activities related to drug therapy to enhance personal knowledge as well as that of other health professionals Participates in administrative, medication use evaluations, and other performance improvement activities Demonstrates regard for dignity, respect, and safety to all patients, family members, visitors, and hospital personnel, as defined in the philosophy of the organization Participates in meeting the goals and objectives of the department to ensure the attainment of the hospital's goals and objectives Controls the drug use process in the patient-care area by working closely with nursing and medical staff to ensure cost-effective use of drug therapy, appropriate drug administration, and efficient drug distribution Suggests ideas for cost control Assumes responsibility, accepts accountability and makes decisions related to the provision of pharmaceutical services Dispenses medications accurately Reviews orders to verify and confirm accuracy and appropriateness of drug, dosage, and route, and to detect incompatibilities of drug interactions Identifies problems related to aspects of drug use control and offers constructive ideas for resolution Duties & Responsibilities Ensures that policies and procedures are followed Provides safe and accurate preparation and dispensing of cytotoxic agents Satisfies performance and quality control standards established for departmental clinical programs Monitors drug therapy of targeted patients by direct involvement in the patient care area and routine evaluation of the patient's drug regimen, medical problems, laboratory data and special procedures Communicates relevant findings and recommendations to other health care practitioners Provides verbal and/or written consultations in the areas of drug therapy selection, pharmacokinetics dosing, nutritional support etc. Participates in the management of medical emergencies, adverse drug reactions, acute and chronic Detects and reports adverse drug reactions according to the hospital's adverse drug reaction reporting system Assists nursing in discharge planning and patient education related to the appropriate use of medications when requested Provides the medical and nursing staffs with information regarding appropriate cost- effective drug therapy and drug administration Provides drug information and educational programs (inservices) to the medical and nursing staffs, other health care professionals, and support staff Participates in the orientation and education of pharmacy students, and new department employees Maintains professional competence by attending and presenting conferences, attending continuing educational programs, and reading current professional literature Supervises technicians in drug dispensing and distribution duties Ensures that technical duties are performed accurately and efficiently Provides ongoing feedback to technicians regarding their performance Provides objective verbal/written information to the pharmacy director on the performance of technical staff Ensures necessary records are appropriately maintained according to legal requirements and institutional policies and procedures: Controlled drugs; Investigational drugs; Productivity measures; Dispensing records; Billing records Ensures the pharmacy service areas and drug storage areas are kept clean and orderly Participates in planning and implementing change and maintaining and improving productivity through attendance and participation in staff meeting, committees, task forces, projects, and discussions with supervisor. Ensure Hazardous Waste or Hazardous Waste Pharmaceuticals are discarded, stored, and managed with other compatible waste streams in containers that are properly labeled and closed prior to being shipped offsite for proper disposal. Initial RCRA training is received within 6 months of the hire date and continuing with annual RCRA training.
