Full-Time | Monday – Friday | 8:00 AM – 5:00 PM

(No weekends!)

‍⚕️ What You'll Do

As a Patient Care Manager, you will:

✔ Lead and supervise clinical staff to deliver top-quality patient care

✔ Coordinate and oversee individualized patient care plans

✔ Ensure compliance with Medicare & regulatory standards

✔ Collaborate with patients, caregivers, and healthcare providers

✔ Drive performance through quality improvement initiatives

✔ Mentor and develop staff through coaching and education

✔ Promote a culture of excellence, safety, and compassion

✅ What You Bring

• Active Registered Nurse (RN) license

• Home Health experience

• 2+ years of leadership/supervisory experience preferred

• Knowledge of Medicare guidelines & regulatory standards

• Strong leadership, communication, and organizational skills

Pay Details: $10,000.00 to $11,000.00 per year
