Speech Language Pathologist

Overview Magnolia Post Acute Care is seeking a Speech Language Therapist to provide rehabilitative therapy to patients with communication, cognitive, and/or swallowing disorders. The role is PRN or full‑time, starting pay up to $60 DOE. Responsibilities Screen/evaluate patients with communication, cognitive, or swallowing disorders and develop an appropriate plan of care following regulatory and clinical practice standards. Utilize standardized assessments and evidence‑based practice to support clinical interventions. Provide rehabilitative, skilled, and medically necessary treatment interventions to patients per regulatory requirements. Assess patient progress, adjust treatments, evaluate treatment outcomes for effectiveness, and modify services accordingly. Offer consultation and counseling to patients, families, caregivers, and other service providers. Communicate patient progress, barriers, and treatment plans to supervisor and health team members. Provide comprehensive discharge summaries of services per regulatory requirements. Document all regulatory and clinically required information in the electronic documentation system and maintain patient records per company policy. Accurately record treatment minutes for services provided. Participate in patient care conferences, utilization review meetings, rehabilitation meetings, and other required trainings, in‑services, and conferences. Obtain necessary DME or supplies per regulatory and clinical practice requirements. Supervise and support CFY or Student Speech‑Language Pathologists. Adhere to ASHA and State required practice acts, code of conduct, ethics during professional practice. Perform other duties as may be assigned. Qualifications Current, unencumbered, active license to practice as an SLP in California (ASHA membership required). Current CPR certification (American Heart Association) and BLS familiarity with assisting clients/residents with activities of daily living and taking vital signs. Experience with regulatory and clinical practice requirements for rehabilitation services. Excellent communication skills and ability to work collaboratively with a multidisciplinary team.
