Job Summary

Caregivers at Graceful Aging Home Care provide compassionate and professional services to individuals in their own homes who require assistance due to aging, illness, disability, or other conditions. This role is essential in helping clients maintain their independence and quality of life, while ensuring services are delivered with care and respect, in line with company policies and industry standards.

Graceful Aging Home Care serves clients throughout Indiana, with our home office located in Indianapolis.

Job Responsibilities

• Follow the client’s written care plan to ensure all needs are met.
• Assist clients with Activities of Daily Living (ADLs) , including:
  • Bathing, toileting, dressing, grooming, and mobility assistance.
• Maintain client safety and security by supervising and ensuring a clean, hazard-free home environment.
• Provide companionship , including social interactions, meaningful conversations, and emotional support.
• Encourage activities that promote mental stimulation and engagement.
• Plan and prepare meals according to the client’s dietary needs and preferences.
• Perform light housekeeping duties, such as changing linens, vacuuming, laundry, and general tidying.
• Provide medication reminders to ensure adherence to the care plan.
• Assist clients with transportation to appointments and errands, including grocery shopping, pharmacy visits, and banking.
• Maintain accurate and up-to-date client records and documentation as required.
• Perform other duties as assigned to meet client and company needs.

Required Knowledge and Skills

• Proficiency in personal care and home management skills.
• Knowledge of client-centered care principles, including assessing needs, meeting quality standards, and ensuring client satisfaction.
• Ability to communicate effectively in English , both verbally and in writing.
• Basic clerical skills, including maintaining client records and completing required forms.
• Strong organizational, problem-solving, and time-management abilities.
• Demonstrated reliability, professionalism, and respect for clients’ dignity and independence.

Qualifications

• High school diploma or equivalent.
• Current CNA, HHA, or QMA license (required).
• Valid driver’s license with reliable transportation preferred but not required.
• Auto insurance that meets state requirements preferred but not required. 
• Must pass a drug screen and background check.
• Minimum 2 years of caregiving experience preferred but not required.
