Health Navigator

The Health Navigator supports patients by coordinating the non-clinical “next steps” that keep care moving—appointments, follow-ups, referrals, and practical resources. This is a highly operational, patient-facing role focused on clear communication, strong follow-through, and accurate tracking to support continuity of care. About Prism Health Prism Health Lab is a community-based medical practice that has served Chicagoland residents since 1987. In response to the global COVID-19 pandemic, we pivoted our practice to support local communities and have made significant strides by focusing our efforts on COVID-19 specific services. To date, we have tested over 50,000 people.We’ve created eight permanent locations, each of which has an incredible story and was offered as a gift by the communities we serve. We’ve also built mobile teams that can work across the State, ready to be deployed as new hotspots arise. We’re dedicated to our neighbors and have partnered with a variety of organizations and leaders including local and County Health Departments, school districts, the City of Chicago and its Aldermen, food banks, Brookfield Zoo, and professional organizations like the Chicago Medical Society. Description Health Navigator (On-Site | Chicago, IL) Hourly Rate: $17–$19/hour Position Type: Hourly (Full-time or Part-time based on program needs) Schedule: Rotating shifts to support extended hours/coverage as needed Location: On-site (Chicago, IL) Key Responsibilities Track patient follow-ups, referral status, and next steps; close gaps through timely outreach Schedule/confirm/reschedule appointments; provide instructions, reminders, and readiness support Identify barriers to care (transportation, scheduling, access) and help resolve them Coordinate referrals with internal teams and external partners while protecting confidentiality Connect patients to resources (transportation guidance, community referrals, practical support services) Conduct structured outreach (calls/texts/emails) to enroll, re-engage, and reduce missed appointments Document interactions and outcomes in trackers/systems (Google Sheets, CRM/EHR as applicable) Support day-to-day operations tied to patient flow (shared inbox/phone coverage, rosters, basic materials) Other duties as assigned to support program needs Qualifications 1+ year experience in patient navigation, care coordination, referral management, scheduling, case support, or healthcare/community health administration Strong organization and follow-through; able to manage high-volume outreach with accuracy Comfortable in fast-paced environments with shifting priorities and time-sensitive needs Strong documentation skills; proficiency in Google Workspace (especially Sheets); able to learn CRM/EHR workflows Clear, patient-friendly communicator who collaborates well across teams Preferred Experience with community-based services and referral networks (e.g., WIC, Medicaid enrollment support, housing/shelter resources, transportation supports) Prism Health Lab is an equal opportunity employer and is committed to building a diverse, inclusive workplace free from discrimination and harassment. #J-18808-Ljbffr Prism-Health
