Holiday coverage may be required.

Bachelor's degree in Physical Therapy

Licensed Physical Therapist in the state or hold an unencumbered compact license

Minimum of one year of experience as a Physical Therapist in a home healthcare setting

General Certifications (BLS/BCLS)

Rehabilitation Certifications (ASHA, CHT, ACP Training)

Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.

Perform treatment programs according to agency policies and standards of care.

Communicate and collaborate with members of the healthcare team.

Document evaluations, treatment goals, and objectives.

Educate patients and family members on the rationale for treatments and procedures.
