Home Care Registered Nurse

Shift Information

Shift: Sunday - Thursday, 8am - 4pm. On-call rotation and holidays required.

Job Description

The Home Care Registered Nurse is responsible for providing comprehensive nursing care to patients in their homes, ensuring high-quality and evidence-based practice. The ideal candidate will perform the following duties:

Complete OASIS documentation in alignment with agency policies and federal regulations, maintaining current knowledge of documentation requirements.
Assess patient and caregiver needs using the nursing process to facilitate the delivery of quality care.
Develop and communicate realistic plans of care that involve patients and caregivers in the decision-making process.
Act as Case Manager for assigned patients, effectively managing caseloads and visit schedules to meet agency productivity standards.
Implement medical plans of care and nursing interventions, adhering to established safety and quality care standards.
Stay updated on high-tech nursing skills including IV maintenance and complex wound care while ensuring infection control standards are met.
Collaborate with Licensed Practical Nurses (LPNs) within their scope of practice, ensuring effective care delivery and task delegation.
Evaluate the effectiveness of the plan of care by monitoring patients' responses and adjusting care as necessary.
Maintain accurate and up-to-date documentation of nursing care to ensure seamless communication within the healthcare team.
Follow agency protocols for documentation and visit procedures, ensuring visits are completed timely.
Provide education to patients and caregivers to empower informed decision-making regarding care and treatment options.
Identify interdisciplinary needs for patients and communicate them effectively to ensure comprehensive care.
Complete required forms accurately and in a timely manner during patient visits.
Conduct visits efficiently, ensuring supervisory visits are not combined with routine patient visits unless necessary.
Assess patient needs for Home Care Aide (HCA) services and develop comprehensive care plans accordingly.
Report patient incidents to quality improvement specialists as necessary.
Coordinate safe patient discharges and transfers, ensuring all paperwork and patient information are processed correctly.
Collaborate with the healthcare team, including physicians and vendors, to ensure safe and effective patient care.
Participate in monthly peer review audits, providing and receiving constructive feedback professionally.
Facilitate case conferences with team members to enhance communication and collaboration in patient care.
Advocate for patient care issues, ensuring that the quality of care is sensitive to the specific needs of each patient and family.
