RN/LPN Restorative Nurse Little Village Nursing and Rehabilitation Center We are seeking a dedicated and compassionate RN/LPN Restorative Nurse to join our team at Little Village Nursing and Rehabilitation Center, a 106‑bed skilled nursing and psychiatric facility located in the historic Little Village community in Chicago, Illinois. As a Restorative Nurse at our center, you will play a vital role in providing high‑quality, patient‑centered care to our residents. Summary As a Restorative Nurse, you will work closely with our interdisciplinary team to develop and implement individualized care plans for our residents. Your expertise will ensure that our residents receive the highest level of care, focusing on their physical, emotional, and social needs. You will assess, plan, implement, and evaluate the care of our residents, utilizing your knowledge and skills to promote their independence, dignity, and well‑being. Key Responsibilities Assess and analyze the needs of our residents, developing individualized care plans to promote optimal outcomes Provide direct care to residents, including wound care, tracheostomy care, and pressure ulcer prevention and management Collaborate with other members of the interdisciplinary team to develop and implement comprehensive care plans Educate and support residents, families, and caregivers on care plans, medication, and treatment options Document resident care and progress accurately and thoroughly in accordance with our center’s policies and procedures Participate in quality improvement initiatives and activities to ensure the highest level of care at our center Requirements Current RN or LPN licensure in the state of Illinois 0‑2 years of experience in a long‑term care or rehabilitation setting Ability to work effectively in a fast‑paced environment, prioritizing tasks and managing workload efficiently Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and colleagues Compassionate and empathetic approach to patient care, with a focus on exceeding resident expectations Why Choose Little Village Nursing and Rehabilitation Center?
