Job Summary

The RN Healthcare Coordinator (RNHC) is a key member of the multidisciplinary team responsible for coordinating, monitoring, and supporting the healthcare needs of individuals served in Day Services. The RNHC blends direct clinical oversight with programlevel coordination, ensuring high-quality, compliance, and person-centered healthcare delivery. This role requires a MA registered nursing License.

This role partners with day service leadership and nursing staff to oversee medical care, support staff training, ensure regulatory compliance, and act as a liaison with healthcare providers. The RNHC provides both hands-on clinical guidance and leadership support to ensure continuity and excellence in care.

Essential Duties and Responsibilities Clinical Coordination & Direct Support

• Coordinate and oversee healthcare services for individuals in assigned day service programs, including medical & behavioral care.
• Conduct and review health assessments, review physical exams, physician orders, and treatment plans; ensure timely follow-up and implementation.
• Monitor and maintain accurate medical records and documentation in compliance with regulatory and agency standards.
• Serve as a liaison with physicians, hospitals, pharmacies, and other healthcare providers to support continuity of care.
• Provide guidance on health-related concerns, problem-solving, and care planning for individuals.

Nursing & Staff Support / Supervision

• Provide clinical support and supervision to LPNs and day service staff in assigned programs.
• Serve as a mentor and clinical resource to nursing staff and assist with onboarding and training.
• Participate in hiring and development of nursing staff as needed.
• Collaborate with Program Directors and interdisciplinary teams to ensure consistency and quality of care.

Medication Administration

• Conduct quality assurance reviews of medication systems, documentation, and administration practices.
• Ensure adequate medication and medical supplies are on hand.
• Monitor medication occurrences and ensure appropriate reporting and follow-up.  

Quality Assurance & Regulatory Compliance

• Review medical records, treatment protocols, and documentation to ensure compliance with state, federal, and funder requirements
• Participate in audits, quality improvement initiatives, and healthcare planning activities.
• Ensure adherence to infection control standards, safety practices, and universal precautions.
• Assist in development and implementation of healthcare policies and procedures.

Training & Education

• Provide health-related training and in-service education to day service staff (e.g., vital signs, chronic conditions, infection control).
• Educate individuals served and staff on medical conditions, treatments, and wellness practices.
• Support training on electronic health records and healthcare protocols.

Team Collaboration & Leadership

• Act as an integral member of the interdisciplinary team, supporting personcentered planning and outcomes.
• Facilitate communication among nursing, residential, and clinical teams.
• Support program operations by providing coverage or consultation across all day service programs as needed. Travel required.

Required Education & Experience

• Licensed Registered Nurse (RN) in the state of Massachusetts required 
• Minimum of 2–3 years of nursing experience, preferably in community-based or residential services.
• Experience supporting individuals with developmental disabilities, mental health needs, or complex medical conditions preferred.
• Prior supervisory or leadership experience preferred.  

Skills & Competencies

• Knowledge of Mass Health, DPH regulations, and community healthcare systems
• Effective communication and teaching ability
• Ability to lead, mentor, and collaborate across teams
• Strong organizational and documentation skills

Licensure & Additional Requirements

• Current MA Nursing License (RN)
• CPR certification
• Valid driver’s license and reliable transportation

Physical Requirements

• Ability to lift, push, and maneuver up to 25 pounds
• Frequent standing, walking, and stair climbing
• Ability to travel between programs
• Visual and auditory acuity sufficient for clinical tasks and documentation
